Case 02 : Preventing Stroke In A 85 Year Old Man

The patient was an elderly 85 year old man from Baroda, who had complaints of repeated episodes of weakness of the right side of the body since the last 2 months. He had two episodes of transient hemiparesis of the right side of the body, recovering within 2 to 3 days. He had a severe stenosis in the left internal carotid artery which was successfully treated with angioplasty and stenting.
This case demonstrates the safety and efficacy of a carotid angioplasty, in well selected patients suffering from carotid artery stenosis.

Case 03 : Case of Recurrent Stroke

73 year old lady, case of diabetes mellitus and hypertension, on treatment, presented with recurrent episodes of transient weakness of the left upper and lower limbs.
She was clinically diagnosed to have TIA ( Transient Ischemic episodes ) .

Carotid artery Doppler ultrasound study revealed a severe stenosis of the right internal carotid artery.
Digital Subtraction Angiography confirmed the finding as shown in the upper picture.

Through the right femoral artery, a 8 French guide catheter was percutaneously placed under local anesthesia into the right innominate artery. A 0.018" Roadrunner guide wire was used to cross the critical lesion in the carotid artery. A 4 mm balloon predilated the lesion, and subsequently a 10 mm x 40 mm Wallstent was deployed across the lesion. There was an excellent result, with no residual stenosis at the site of original narrowing. The patient was discharged from hospital the next day.
